IHL and IOW NHS Trust Collaborate for a New Community Care Service

The National Health Service (NHS) on the Isle of Wight is facing a crisis, with reports indicating a strain on hospital beds and resources. In response to this, Island Healthcare has partnered with the Isle of Wight NHS Trust to offer a new and unique care service in the South Wight area. The Island Healthcare Community Hub, located at Tile House in Shanklin, is designed to support patients who have been discharged from NHS community beds and help them maintain their independence and reach their potential.

The Community Hub offers two main services: the Day Support Hub and the Home Care Hub. The Day Support Hub provi

des a supportive and engaging environment for patients, offering activities to improve dexterity, opportunities for socialisation, and prepared meals. Transportation can also be arranged if needed. The Day Support Hub is open from 10.00am to 4.00pm.

For times when the Day Support Hub is not available, the Home Care Hub offers support in the comfort of patients’ own homes, with up to three visits a day including meals.

Access to the Island Healthcare Community Hub is facilitated through referrals from the NHS and is free of charge. the Tile House Community Hub reflects the current service currently being offered privately out of Tile House, the Hub and was created to support hospital discharge at a time when the NHS on the Isle of Wight is facing challenges and to provide high-quality care for patients in the South Wight area.

The collaboration between Island Healthcare and the Isle of Wight NHS Trust is an important step in addressing the crisis faced by the NHS on the Isle of Wight. By providing high-quality care and support to patients who have been discharged from hospital, the Island Healthcare Community Hub aims to alleviate some of the pressure on the NHS and help patients achieve their best possible health outcomes.
